Forms Central

The ABTC uses several forms to assist various club functions. Most of the forms are available here for download, and must be printed because they require manual signature or enclosure of payment. A few can be submitted on-line through direct e-mail from an ABTC Website page. Adobe Acrobat ReaderŽ or other software will be required to view or print the downloadable forms.

Rescue:

	Guide for Adopting a Belgian Tervuren. A tri-fold pamphlet for printing and public distribution.
	Adoption Application pdf form for mail-in to: Cindy Simonsen, W4163 Cty ES, Elkhorn, WI 53121. Requires manual signature.
	Adoption Contract pdf form for information only. Not valid without completion by the Belgian Tervuren Rescue, Inc. with signatures of all parties.
	Rescue Guide for Impounded Belgian Tervuren . A tri-fold pamphlet for printing and distribution to animal shelters and other rescue organizations.
	Volunteer/Foster Care Application pdf form for completion and mail-in by ABTC Member volunteers to help and provide foster care. Requires signature.
	Release Form pdf form for legal transfer of rescued Terv to Belgian Tervuren Rescue, Inc. Requires signatures of all parties.
	Rescue Expense Reimbursement Request pdf form for Members authorized to assist in rescue activities. Requires signature and receipts.

General Store:

	Order Form for merchandise from the ABTC General Store. Payment by check or money order.
	2006 National Specialty Order Form. A limited quantity of select Specialty merchandise is available.

Membership:

	Associate Membership Application pdf form for persons first applying for ABTC membership. Requires signatures of applicant and sponsors.
	Regular Membership Application pdf form for application by ABTC Associate Members who have completed one year or more of membership in good standing. Requires signatures.

Area Clubs Forms:

Area Club Request for Show Insurance coverage . On-line form e-mails directly.

Club Treasurer:

Request for expense reimbursement pdf form by Members for authorized Club business. Requires signature and receipts.

Awards:

Member Awards Request Form. Requires signature and copy of award by February 28th. each year.

Junior Handling:

To obtain recognition for participation in Junior Handing events.

Perpetual Achievement Lists:

Templates for publishing lists on this Website from MicrosoftŽ Word, Excel, and Works file formats; and HTML. May be sent in as e-mail attachment.

Breeder Information Committee:

Application form for ABTC Members to volunteer to assist people seeking a Terv. Signatures and references required.
